There was a Near Miss in the East Crank line recentlyat the Crank Final Unload station. The overhead hoist trolley wheels broke on one end. See pictures above. The hoist did not fall as the secondary restraint was still connected and the wheels were still attached on the other end. Please note – the spacers for balancing the hoist were offset. The spacers should have been aligned as identified by the green arrows. Being misaligned made the hoist harder to move and put it in a bind. The rubber stop on the cushion was also missing. Please share with all and especially with the Millwrights that do the overhead monthly hoist inspections.
